About the Testing
Center
The CBA Testing Center in Room 36 CBA was conceived and designed by the College of
Business to provide a service to CBA instructors and students. Starting in Fall 2011,
a proctored testing environment is available for make-up exams, exams for online CBA
courses, and the Major Field Test in Business assessment exam. Please see the Testing
Center Policies document below for important information on using the Testing Center.

Summer 2013 Testing Center Hours
The CBA Testing Center is open from May 21st through August 15th during the posted hours. Testing is not provided in the CBA Testing Center on Mondays or on weekends. The only exception to this will take place on the first week of July, during which the Testing Center will be open on Monday, July 1st from 8:30a to 2:30p, as well as Tuesday and Wednesday (during the normal hours for those two days). It will be closed on July 4th and 5th, due to Independence Day.
In case of a weather or other type of emergency that results in classes being canceled, the CBA Testing Center will be closed.
Procedures
Instructors wishing to make use of the CBA Testing Center should familiarize themselves
with the testing center policies, and complete the appropriate forms. For more information

Testing
Center Policies Document
All instructors using the testing center should make themselves familiar with these
policies before using the testing center.

Request for Exam Services
All instructors who plan on using the testing center need to complete this Request
for Exam Services form. This form provides us with instructor contact information and
information about the exam(s) that students will be taking. This form needs to be completed
for both make-up exams for individual students and semester-based exams for online courses.

Request for Exam Changes
If an Exam Request has already been submitted and approved, changes must be submitted
using this Request for Change form. Changes will be evaluated and approval will
be based on other exams previously scheduled.
